Refresh package icon seems redundant

Based on study feedback, users felt the refresh packages icon might be redundant since edits seemed to update in the panel immediately and there is a refresh environments button that would do the same thing.

We could consider removing this

@karthiknadig do we have this to handle installing packages from the command line and then refreshing to get the update from there? I remember there was a reason behind this button and unsure if thats still the case.

Ah I just tested and the list doesnt auto-reload for packages installed from the terminal. Is this something we could enable?

It should work, if you do it from Powershell. but we can't detect this from cmd.

There is a more general fix, we can monitor the site-packages and update whenever there is a change there.

as a fix for MVP does it sound good to drop refresh into a sub-menu:

Let's put this on the backlog unless we might be able to get to it prior to MVP. I think this flow is more delightful than relying on a refresh button.
