Create a cmd top level directory and move main.go there

What would you like to be added:

A cmd directory at the top level, and move to it the main.go file we currently have in pkg/ext-proc

Why is this needed:

To match the recommended golang repo structure, and will allow us to host multiple extensions, for example #321
