Git Web UI: Local and Remote branch buttons are not accessible

Neither the "Add" Local Branch button, nor the "Refresh" remote branches buttons are accessible:

1. When hovering over the buttons, a tool-tip should appear that states the expected action of the button. (My primary reason for opening this issue)
2. When tabbing through the UI, these buttons should have a focus state.

(really, anything clickable/actionable should have a focus state)

Along these same lines of accessibility, when I add a new branch and my cursor is in the new text box, I would expect that hitting tab should take me to the check mark icon. However, focus ends up on the refresh remote branches icon/button. (Which is not obvious because the refresh icon does not have a focus state)

I have to tab backwards to get to the check mark button, which is counter intuitive. (although the check mark icon does gain a focus state.)
