Fixes #1582 and #1583. Previously, macros declared in non-documented symbols weren't being added to the table. Adds tests for that case, places guard asserts to make sure we don't try to resolve macros before we have the macro table populated, and adds a new warning for when a user creates a macro reference that doesn't have a corresponding definition.
