Maintenance: Revisit issue forms

Summary

This repo uses issue forms, we should revisit the forms to make some small fixes:

• Move this message to the bottom of the issue so that it appears in the opened issue instead of only in the form:
  

• Add .NET to the "Acknowledgement" section

• Review all field types

Why is this needed?

To maintain the form and for them to be useful
